This form is provided as a service by the St. Catherine of Siena School Athletic Boosters Club. The Coaches Committee of the Boosters wishes to have feedback on the performance of your child's coach(es). This performance evaluation will help us to better assess each coach's strengths and areas in need of improvement. Your candid response with objective examples where possible is greatly appreciated. You are encouraged to discuss this form with your child. The responses will be reviewed and summarized by the Coaches Committee and then shared with the coach. The Boosters strive to provide a means for our school children and adults to learn and practice good sportsmanship, honesty, integrity and respect for rules, within an atmosphere of Christian principles. To that end, the Boosters have developed and supervise a comprehensive program of sporting activities. The best interest of the children of St. Catherine of Siena School and their Christian development shall always be the guiding principle in matters concerning the Boosters and its activities.
